We will have agreed with you the outcomes that matter to you, the support that you will receive, and estimated timescales for these actions, if applicable.

We will set a review date with you – this is a date on which we will review your Adult Carer Support Plan to see how things are going for you at that stage.

There are also additional circumstances in which your Adult Carer Support Plan will have to be reviewed. These are:

• If you move to another area outside of Perth & Kinross.
• If the person you care for goes into care.
• If there are changes to your circumstances and you feel this has a material impact on the care you provide.
• If there are changes to the circumstances to the person you care for and you feel this has a material impact on the care you provide.
• If you no longer feel you are able or willing to continue to provide care.
