The Adult Carer Support Plan is essentially an informal conversation between you and us. After we have contacted you to arrange a mutually convenient time and place to meet, we will discuss your needs as a carer and how we may help to support you in your caring role. Although you can feel free to tell us anything, to help you feel more prepared for the meeting, these are the types of questions we will ask you:

• What caring tasks you do and how you feel about doing them.
• Whether you feel able and willing to continue caring and whether you feel your caring role is sustainable.
• How caring is affecting your mental and physical health, and emotional wellbeing
• How caring is affecting your relationship with the person you care for and others
• How caring is affecting your education or work, your ability to go into these, or if there is any training you think would be beneficial for you or the person you care for to improve independence.
• How caring is affecting your living or caring environment.
• How caring is affecting your finances.
• How caring is affecting your work/life balance.
• Your hobbies and interests, hopes and dreams.
• What you think would help to make things easier for you.

We will discuss the outcomes that matter to you and the support that will help you in your caring role as well as help you to have a life alongside caring. Before your Support Plan is completed, we will ask you if you agree with it and you will receive a copy for you to keep. This is so that you know what to expect going forward.

We will also set a review date with you – this is a date on which we will review your Support Plan to see how things are going for you at that stage.
